Who is eligible for children's CHIP/Medicaid in Idaho?

To be eligibility for the children’s CHIP/Medicaid program in Idaho, a child or teen must meet three requirements:

1. Age – a child must be under 19 years old
2. Citizenship / Legal Residency Status – a child must be a U.S. Citizen or Legal Permanent Resident
3. Income – the child’s family income must be within certain income limits

This document includes a 1/2 page English and 1/2 page Spanish screening tool that can be used in clinics, or it can be altered to use in schools, churches, or any other community location where families will have access. Below is an example of the English screening tool:
